We don't observe such behavior in current software version with genuine LKP-02 transponders:
You can try to format your transponders first via Transponder Type->Emulators->LKP-02
If after that you still can't write the pages, then likely the transponders you got are defective or fake.

As already stated we don't observe such behavior with LKP-02 (tried transponders from multiple batches).

Try to place the transponder at different locations, all around the deeper slot for the remote, not just place the transponder at the carbon/plastic and glass slot that dedicated for transponder. I have noticed my Tango seem to get a bit weak now from reading/writing and if I don't place the transponder at the sweet spot then it will have incomplete write which is random errors at the end. I just clone a smart key 2012 Lexus RX450h to LKP-02 (Tango V1.137.1) perfectly fine yesterday. The sweet spot on my Tango is the top-right corner.
I finally received a request to create a spare D1 key and carried out the work.
I was able to register it without any issues using a reset used key.

2018年以降のトヨタ車に関しては、「全キーミス」のプログラミングやキーの追加といった、以前は問題なく認識していた作業ができなくなりました。

In my case:
After updating to version 1.137/1.137.1,
I am no longer able to perform tasks on Toyota vehicles (2018 or newer) that previously worked without issues—specifically, "all keys lost" programming and adding new keys.
(BA) Prius 60 / Yaris Cross / Corolla Fielder
(AA) RAV4
I am also unable to perform "all keys lost" programming for the Prius 50.
Is everyone else able to do this without problems?
Using the latest version of SLK-08.

I attempted to add a smart key using the SLK-08, but it was unsuccessful.
The same error occurred when using the SLK-07. I have attached an image of the error screen.
[Questions regarding SLK-08 usage]
1. Is a reset required after using the SLK-08?
2. When the initial button ("Connect to Vehicle" / "Learn Key") is pressed, a message appears prompting you to press the Start button; regarding this... you can turn on the ignition using a currently valid key.
3. Afterward, the display shows "SW IG ON with SLK08---", but even when holding the SLK-08 close to the Start button and pressing it, the ignition does not turn on.
